Grealish, 27, arrived at the Etihad for £100million in August 2021 and won the Premier League in his first season but has struggled to impress on a regular basis.
The former Aston Villa man grabbed two assists as the champions returned to Premier League action with a 3-1 win over Leeds last night.
But his tally since joining City stands at only four goals and five assists in 35 league appearances.
The report indicates Pep Guardiola is willing to let the England international leave in favour of bringing Leao in.
The Rossoneri winger is one of Serie A’s most in-demand talents after notching seven goals and nine assists in 20 games this term, as well as two strikes for Portugal at the World Cup.
And the former Sporting and Lille star has admitted he is open to a move to the Premier League in the future.
On whether a move to the English top flight attracts him, he said: “Yes, in the future.
“But now, I’m 100% focused on Milan. I’m at a top club and I’m under contract. I also like the city.”
